5. Go to Hospital Kerajaan/Klinik Swasta for body Check-UP
- 1st time : Bring along your Borang Laporan Pemeriksaan Kesihatan & Documents related
- There will be Another appointment date for you to get the completed borang with X-ray film (make sure the doctor has signed/chop/sahkan the borang)
- Penang juniors please go to 001, Counter H which opens at 9.30am, pay RM20, filling the forms given by Hospital & take urine test + eye,blood pressure test + x-ray, there will be another appointment date for u go to again
6. Go to Bank Islam Malaysia Berhad to open one New account with ATM card
- Isikan kedua-dua keping Kad Tandatangan Spesimen - Simpanan & Pelaburan (photo below- the small card)
- Isi borang pendaftaran untuk perbankan SMS Bank Islam (not compulsory)
- If you are not applying PTPTN/JPA, Don't need to open account tetapi kalau ada apa-apa pembiayaan, bendahari UKM akan salurkan melalui BIMB
- If you already have a bank islam account, then don't need to open account anymore
- If you are applying PTPTN & JPA, you will need a bank islam Account
- For PTPTN applicants, you can open your SSPN account at bank Islam. For more info, please view http://www.ptptn.gov.my/web/guest/simpanan/borangsspn (Tips for SSPN form : write your details at butir deposit & butir penerima, write ur dad/mom's details at butir penama)
- Remember to tell them that you just want an Akaun simpanan Wadiah (RM30 deposit) or they will ask u to deposit RM130.
- No bankbook given, they will give you a slip after you open the account.
- Remember to register internet banking too at atm card machine after u opened the account by creating internet banking PIN (different from your Pin for atm card) so that you can log in to the internet banking www.bankislam.biz

7. Make payment for yuran kemasukan at Bank Islam
- There are 3 slips - 1st slip will be kept by bank, u have to Keep the 2nd & 3rd Bill Payment Slip and Bring it to UKM During Registration
- yuran kemasukan does not include yuran pengajian, you can pay the yuran pengajian after you enter ukm
- You Don't Need a Bank Islam Account to make Payment, Just go to the counter, tell that you want to pay for yuran kemasukan UKM , they will guide you. Fill in the Bill Payment Slip, Bring along your Money & Documents related

- Suggestion : It's better to get Formal photo (means tie your hair for girls, wear formal attire, no t shirt/collar T), so that you no need to retake photo
- Informal photo (no tie hair, v t-shirt/Collar T) also can, as long as it is clean & tidy
- bring your photos to ukm, it's better to have softcopy/disc for your photo
9. Have your own Document File which contains SPM Cert, MUET cert, STPM/Matrik cert, Surat Tawaran & etc with pengesahan&signed &chopped.
10. Wait for Your PARCEL ^.^
- you will need to prepare all the documents(with pengesahan) stated in the fail pelajar & arrange accordingly
- bring the 2nd & 3rd payment slip for yuran kemasukan & passport photos
- fill in the info needed at Fail Pelajar
- you will get a pelekat filem X-ray, stick it at sudut atas sebelah kiri X-ray envelope, not at the X-ray film. (if scared do wrong then bring it to UKM and ask the PC where to lekat during registration)
- car sticker is for juniors who r coming with parents/relatives, put the car sticker at your parents' car to enter UKM entrance.

ReplyDeleteAccording to the info I get from UKM, there will be no buku pendaftaran pelajar in the parcel, so u might need to print it out as stated in the pendaftaran web.. but u will use it a few times only for reference.. (my opinion)>>print out the important ones if you want to save $$ & paper.

i think is hand in to the bank, they will tell you what to do when you tell the counter that you want to open an account. after you open the account, you will receive a slip(a replacement for bankbook since there is no bankbook),a copy of cash deposit slip & atm card.

1. How long to get the report : U need to go at least twice, 1st time is body check, 2nd time is get ur report, u can get the report at the 1st time. Last time i get my report within 2 weeks (it depends on the hospital). Btw, I am nt so sure about Lam Wah Ee.
DeleteSince u can only go back 1 day, i suggest u to go to private clinic (u hv to pay more but u can get it faster)
2. You can go there 1st sept, but i think u can only stay outside. My suggestions, come early at 2 sept is the best.
3. there is transport out from UKM : a) Bus 6k/6z which bring u to KTM, Rapid Bus which bring u to Hentian Kajang (a lot of buses there back to Penang, johor, anyplace)

Normally we wear casual. Collar T with long pants. But u Have to wear formal during Presentation and Beginning of Sem.

Formal wear is like formal corporate attire. For girls, formal top v collar & black slack, v formal shoes (high heels/not high heels also can). For guys, formal top v collar, black slack, formal shoes.

DeleteI have open bank islam account ord.But tis few days i juz receive the letter from UKM.letter include the all slip deposit bank .so i nid to use the inside letter slip to fill in and do payment yuran kemasukan isn't?
ReplyDeleteyou may use the slip to pay yuran if u havent paid the yuran kemasukan. Just leave the open account form aside since u have already opened the account

Deletelast night i juz receive the email say the photo of apply ukm card not criteria.but tat is difficult to uplaod photo.now i reuplad ord.The upload expired date is 27th august...so tis new upload one if still can not,and expire ord.so can i pass the photo during register for do the ukm card?and ukm card system problem i nid to contact who?can i know the contact number?
