1. 30 hari SELEPAS tamat pengajian (Tamat pengajian here means WITHIN 30 days AFTER you get you final results slips = SEM 2/ 2011-2012 results slip if you are final year now, or SEM 3/ 2011-2012 if you have extra semester).
2. Can be done online 7 days after you get your final results slip (Pegawai JPA said: The result slip usually comes with a document from Senat of University to prove that your study has completed, which this temporary document can replace transcript that is required for Lapor Diri). Click Modul Pemantauan Pelajar in esila JPA and proceed. If you have transcript by the time you get your last results, then proceed with transcript.
3. If Lapor Diri is not completed in WITHIN 30 days after the final results slip, you will get a WARNING LETTER to complete that in 15 days, after the period, you will be required to pay the "Tuntutan Ganti Rugi" and is announced bankruptcy.
4. If your internship is COUNTED in credits and CGPA and it falls in the last semester of your course, then wait for the last results announced and proceed with Lapor Diri.
(i) If Internship falls in SEM 3 (additional semester after SEM 2 final year), and it is counted in credits and CGPA, goes for the contact in Procedure 5 below, inform that when internship ends and Lapor Diri will be late).
(ii) If internship in SEM 3 is NOT COUNTED in credits/ CGPA, last results should be SEM 2, and Lapor Diri should be done within 30 days after the results announced even though you are in internship now.
*** 5. JPA does not have the most updated info of the final date of your course, as such you ARE REQUIRED to inform them that your course ends at what time, and estimates when you will get your final results slip and when you will Lapor Diri here: amalia.kassim@jpa.gov.my OR 03-88853375/3669 (JPA only have the details that your course ends in Sem 2 of final year, additional Sem 3 in final year/ extention of study should be informed).
6. Documents required for Lapor Diri: Surat Senat UKM as mentioned above OR Transcript (comes together with your final results slip), Bukti Permohonan SPA.
7. *NOTE: TO FURTHER STUDY AND TO EXTEND CONTRACT WITH GOVERNMENT
(a) Documents: Surat rasmi permohonan pelajar (self-written), surat sokongan/ kebenaran penjamin (self written; can combine with surat rasmi permohonan pelajar as one), surat tawaran universiti (surat tawaran master/ PHD etc.), surat tawaran penaja lain (if have - penaja here CANNOT BE from private sectors).
(b) Procedure of LAPOR DIRI must still be done before application of further study, then only the procedure in (a) if you have got offer to further study.

8. If you have any changes on e-mail, MUST inform JPA as it is one of the important tool for communication with JPA. Open e-mail regularly when you have any request/ problem/ procedures with JPA.
9. Change in address should be informed as well.
NOTE: Final results slip here is the LAST exam slip you will receive in your course. SPA = Suruhanjaya Perkhidmatan Awam
