a) Borang Laporan Pendaftaran Diri & Syarat-syarat Kemasukan
c) Laporan Pemeriksaan Kesihatan
d) Surat Tawaran (If u need it urgent for applying scholarship/loans/body check-up)
*Info : Parcel yang diposkan kepada pelajar termasuklah Surat Tawaran Asal, Fail Pelajar, Pelekat X-ray, sticker kereta dan Borang BIMB sahaja.

Go to Hospital Kerajaan/Klinik Swasta for body Check-UP
- 1st time : Bring along your Borang Laporan Pemeriksaan Kesihatan & Documents related
- There will be Another appointment date for you to get the completed borang with X-ray film (make sure the doctor has signed/chop/sahkan the borang)
- Penang juniors please go to 001, Counter H which opens at 9.30am, pay RM20, filling the forms given by Hospital & take urine test + eye,blood pressure test + x-ray, there will be another appointment date for u go to again
6. Go to
Bank Islam Malaysia Berhad to open one
New account with ATM card
- Isikan kedua-dua keping Kad Tandatangan Spesimen - Simpanan & Pelaburan (photo below- the small card)
- Isi borang pendaftaran untuk perbankan SMS Bank Islam (not compulsory)
- If you are not applying PTPTN/JPA, Don't need to open account tetapi kalau ada apa-apa pembiayaan, bendahari UKM akan salurkan melalui BIMB
- If you already have a bank islam account, then don't need to open account anymore
- If you are applying PTPTN & JPA, you will need a bank islam Account
- For PTPTN applicants, you can open your SSPN account at bank Islam. For more info, please view

- Remember to tell them that you just want an Akaun simpanan Wadiah (RM30 deposit) or they will ask u to deposit RM130.
- No bankbook given, they will give you a slip after you open the account.
- Remember to register internet banking too at atm card machine after u opened the account by creating internet banking PIN (different from your Pin for atm card) so that you can log in to the internet banking www.bankislam.biz

7.
Make payment for yuran kemasukan at Bank Islam
- There are 3 slips - 1st slip will be kept by bank, u have to Keep the 2nd & 3rd Bill Payment Slip and Bring it to UKM During Registration
- yuran kemasukan does not include yuran pengajian, you can pay the yuran pengajian after you enter ukm
- You Don't Need a Bank Islam Account to make Payment, Just go to the counter, tell that you want to pay for yuran kemasukan UKM , they will guide you. Fill in the Bill Payment Slip, Bring along your Money & Documents related

8.
Prepare Passport Photos (20 and above)
- Suggestion : It's better to get Formal photo (means tie your hair for girls, wear formal attire, no t shirt/collar T), so that you no need to retake photo
- Informal photo (no tie hair, v t-shirt/Collar T) also can, as long as it is clean & tidy
- bring your photos to ukm, it's better to have softcopy/disc for your photo
9. Have your own Document File which contains SPM Cert, MUET cert, STPM/Matrik cert, Surat Tawaran & etc with pengesahan&signed &chopped.
10. Wait for Your PARCEL ^.^
- you will need to prepare all the documents(with pengesahan) stated in the fail pelajar & arrange accordingly
- bring the 2nd & 3rd payment slip for yuran kemasukan & passport photos
- fill in the info needed at Fail Pelajar
- you will get a pelekat filem X-ray, stick it at sudut atas sebelah kiri X-ray envelope, not at the X-ray film. (if scared do wrong then bring it to UKM and ask the PC where to lekat during registration)
- car sticker is for juniors who r coming with parents/relatives, put the car sticker at your parents' car to enter UKM entrance.
